New York State Police launch an extensive investigation into a deadly homicide that shook the town of Deerpark late Friday night
Deerpark, New York – New York State Police are investigating a homicide that occurred late Friday night at an assisted living facility in the Town of Deerpark, an incident that has left the local community shaken and raised serious concerns about safety within care settings.
According to authorities, the incident unfolded on December 13, 2025, at approximately 11:39 p.m., when New York State Police from the Greenville Barracks, along with officers from the Town of Deerpark Police Department, were dispatched to a report of an assault at The Sentinel of Port Jervis. The facility is located at 2247 Greenville Turnpike in Port Jervis.
Town of Deerpark police officers arrived at the scene first. Inside one of the facility’s bedrooms, officers found a 62-year-old woman, identified as Patricia Winters of Port Jervis, unresponsive. She was pronounced deceased at the scene. Investigators determined that Winters had suffered a stab wound.
Also located inside the same bedroom was James Annear, an 84-year-old man from Port Jervis. Authorities said Annear was found with apparent self-inflicted stab wounds. Emergency medical personnel provided treatment at the scene before he was transported to a local hospital. As of the latest update, Annear remains hospitalized in critical condition.
State Police said the preliminary investigation indicates that Annear and Winters were involved in a verbal confrontation that escalated into a physical altercation prior to the fatal incident. Investigators have not released further details about what may have led to the confrontation, and they emphasized that the investigation is still ongoing.
Despite his medical condition, Annear remains in the custody of the New York State Police. Authorities confirmed that he will be charged with Murder in the 2nd Degree, an A-I felony under New York law. His arraignment will take place once his medical condition allows.
The Sentinel of Port Jervis is an assisted living facility that serves older adults who require daily support. Following the incident, law enforcement maintained a presence at the location while investigators processed the scene and spoke with staff and potential witnesses.
State Police have not indicated whether there were prior reports of conflict involving the individuals connected to this case. Officials also have not announced any immediate changes to facility operations, though such matters are often reviewed after serious incidents.
Authorities are urging anyone with information related to the case to contact New York State Police. As the investigation continues, officials say their focus remains on determining the full sequence of events and ensuring accountability under the law.
